How to create a downloadable PDF?

  2. Build the form in Edit survey or Edit input form

  3. (Optional for survey) Enable Collect demographic questions by adding a new page to the end of the survey in Phase → Phase access and user data → Demographic questions asked to users to place registration questions on the last PDF page

  4. Click Download as PDF (in the Survey builder or Survey form tab)

Limitations on FormSync

Supported vs. Unsupported Question Types

Supported (PDF download): All question types can be printed.

Supported (PDF upload/import): Be aware that while all survey questions types can be downloaded as PDF not all survey question types can be imported back using our scanning technology. The following question types are not yet supported for import:

  • Mapping questions (Drop Pin, Draw Route, Draw Area, ESRI file upload)

  • File upload questions

  • Matrix questions

  • Ranking questions

If your PDF survey template includes these questions, when you upload the template back these question types will be uploaded back as "no answer"

File & Scan requirements

To avoid errors during upload, please ensure the following:

  • Use the unaltered platform PDF template (no layout edits)

  • Keep online form unchanged after PDF generation

  • Scan pages with consistent size and adequate contrast

  • Upload ≤ 100 pages per batch

  • Use one language per upload
